Quality failures are shared problems. In a quality culture, a bug that reaches production is a system failure that the whole team learns from, not an individual failure that gets blamed on whoever wrote the code. Blameful postmortems drive bugs underground. Blameless postmortems surface the systemic conditions that allowed the bug to reach production — and produce concrete improvements to the testing infrastructure that prevent the same class of failure.
Quality metrics are visible to everyone. Pass rates, coverage trends, time-to-detect, and production incident rates should appear in the same dashboards that engineering leaders look at for deployment frequency and cycle time. Quality is a delivery metric, not a separate QA metric.
The two barriers that kill quality cultures before they start
Most teams that fail to build a quality culture don't fail because they don't care about quality. They fail because two structural barriers make testing feel like a tax instead of a tool.
The expertise barrier. Writing good automated tests has historically required knowledge of testing frameworks, selectors, fixture management, and assertion libraries.
